Ohio Health Care Association came to Welcome Nursing Home to honor our beloved Molly Yarber with the Best in Ohio Skilled Nursing Facility Excellence Award! Molly has touched the hearts and careers of everyone that calls Welcome HOME. To work at Welcome is to be apart of a family and at home. Molly hires us and then works with everything she has to support, inspire, and motivate us to be our best. No one deserves this award more than Molly and we are so thankful that OHCA acknowledges what we already know!
CMS 5-Star Rating
Welcome Nursing Home is proud to have earned the CMS 4-star rating. The Welcome team works hard each day to provide good, quality of care for all our residents. Our goal is to meet or even exceed the individual needs of all the residents. It is rewarding to have these efforts be reflected in the strict star rating. We are proud of our entire team that have made this 4-star rating possible.

Inside Visitation
Open Visitation daily from 7 am to 7 pm. Visitors arriving outside of this timeframe must use the doorbell to gain access. We ask that all Visitors and Guests sign in and out at our AccuShield electronic stations.
COVID Numbers are no longer being shared through through the website. If we have active COVID cases in the building there will be signage on our doors. If you have specific questions feel free to ask for the Supervisor or someone in Nursing Leadership.
